When should I complete my intake forms?
Complete your forms after your appointment is confirmed and before your first session. Finishing paperwork early helps your therapist prepare and keeps your first visit focused on care.
Which forms do I need?
Choose the section that matches who is starting care: Adult, Children 11–17 (guardian and minor), Children 4–10, Couples, and Telehealth if you will meet online. If you are unsure, call the office and we will point you to the right packet.
Do I need telehealth forms for in-person therapy only?
Telehealth forms are for clients who will also use teletherapy. If you are meeting only in person, complete your age-appropriate intake forms. If your plan includes online sessions later, add the telehealth forms then.
For ages 11–17, who completes which form?
A parent or guardian completes the Guardian form, and the minor completes the Minor form. Both are required. If teletherapy will be used, also complete the telehealth forms.
Can I print PDF forms instead of completing them online?
Yes where a PDF option is listed (telehealth, couples, and children ages 4–10). Print, complete, and bring them to your first session. Adult and 11–17 packets are online FormDr links.
